Home Telehealth

The Hampton VAMC, Home Telehealth program helps Veterans manage chronic medical conditions from home.  Home Telehealth fosters regular and effective communications between patients and clinicians to improve symptoms and decrease hospital visits. Read more about Home Telehealth.

Hospice POCs

Low Vision Clinic

Pharmacy

Rural Health Initiative Van Based Outreach Program

The Hampton VA Medical Center, Rural Health Initiative team is composed of VA employees who travel to the rural communities throughout northeastern North Carolina and southeastern Virginia to provide VHA health benefits and enrollment assistance to rural Veterans interested in VHA services.  Read more about the RHI team services.

Social Work
