The Hampton VA’s Home Telehealth program helps Veterans manage chronic medical conditions, without leaving home. Telehealth fosters regular and effective communication between patients and clinicians to improve symptom management and decrease hospital admissions and placements in long-term care nursing homes.

Using a simple monitoring device that taps into a telephone landline, Veterans send information — such as their blood pressure, weight, glucose readings and mood — to a Nurse Care Coordinator.  These details enable hospital staff to keep close track of their medical conditions.

The programs serve Veterans with an array of challenges including:

Congestive Heart Failure, Diabetes, High Blood Pressure, Depression, Chronic Kidney Disease, Obesity, Post-Traumatic Stress Disorder, Chronic Pain, Substance Use Disorder, Bi-Polar, Chronic Obstructive Pulmonary Disease (COPD).

Now Veterans with cell phones can also participate in this program using our new Interactive Voice Response (IVR) System.
